Case Presentation
A 23-year-old patient reported with a chief complaint of discoloration and poor esthetics of the upper front tooth, along with concerns regarding smile symmetry. The patient gave a history of trauma to the maxillary anterior region following an accident approximately 7–8 years ago. Clinical examination revealed an Ellis Class II fracture involving tooth #21. The tooth exhibited marked discoloration and did not respond to pulp vitality tests, indicating loss of vitality. Radiographic evaluation showed a well-defined periapical radiolucency associated with tooth #21, suggestive of a chronic periapical abscess. Based on the clinical and radiographic findings, a diagnosis of pulp necrosis with chronic periapical abscess secondary to traumatic injury was established. Considering the compromised pulpal status, discoloration, and the patient's esthetic concerns, a comprehensive treatment plan was formulated. The treatment included Root Canal Treatment (RCT) of tooth #21 followed by post-endodontic rehabilitation with a zirconia crown to restore function, esthetics, and color harmony with the adjacent teeth. The endodontic treatment was completed successfully, and a zirconia crown was fabricated and placed to achieve optimal contour, shade matching, and smile symmetry. The final outcome resulted in elimination of infection, restoration of function, improved esthetics, and enhanced patient satisfaction with the overall appearance of the smile.
